Certificates for families in need of comprehensive eye exams and glasses.
Each gift certificate provides access to eye care and, if prescribed, glasses at no cost from a local VSP network doctor. ( A list of VSP providers within 30 miles will be provided with the certificate)
Patient Eligibility:
1. Have a family income at or under 200% of the Federal Poverty Level guidelines;
2. have not used a VSP program in the last 12 months
3. not have coverage through a private insurer or government program for the eye care services or prescription eyewear covered by the certificate
