Every student must have a physical on file
to participate in any sports next year.
If a student is considering participating for the
2023-24 school year in a sport, including cheerleading,
there is some important information.
Students will need to have physical forms all the way filled out.
They are included below.
Make sure students have all the signatures that are needed.
Students must bring $25.00 cash ONLY.
Please have the exact amount.
If a student is absent on the day scheduled for their class,
they will need to go with one of the other grade levels.
There will NOT be physical make-up dates in the fall.

RE: 2023-2024 KSHSAA Physicals
Sports physicals for the 2023-2024 school year will be conducted May 2 nd, 3 rd, and 4 th .
The Kansas State High School Activities Association mandates that every student who participates in athletic
practice or competition must have a completed signed physical form on file in the school office
before participation is allowed. If you are at all considering participating in any sport in 2023-
2024, please take advantage of this one-time reduced cost offer. Physicals will be conducted
at the Lyons Medical Center at 8:30 AM,
with the district providing transportation to and from. If
for any reason a student is absent on the day, they are scheduled for physicals they may attend
another day. Times and dates are as follows:

6th grade: Tuesday, May 2nd
7th grade: Wednesday, May 3rd
8th grade: Thursday, May 4th
Each student will be required to pay $25.00 in CASH to the Medical Center at the time of
his or her physical. Please have the exact amount.
The enclosed Physical Evaluation, Concussion Release, and Parental Permission Statement
will require signatures by both the student and parent/guardian. In addition, the first page of the
physical form is to be filled out prior to arriving at the Medical Center. Please return all forms
on the morning of your son/daughter’s physical.
