Student Surveys

Nonacademic Tests, Surveys, & Questionnaires

 We thank you for being our partners as we work together to provide the highest quality education to our students. The district has partnered with Panorama, FastBridge - MySAEBRS, Gallup Poll, KCTC, and Monitoring the Future which are student surveys and social-emotional platforms. These surveys were created to measure social-emotional concepts.

Students will participate in these surveys to better perceive and reflect on Goals and Challenges, Current Class Performance, Overall School Performance, Behaviors, and Feelings. These are digital surveys and will be completed during a scheduled class time. The surveys vary in length with most taking 15-30 minutes (KCTC is a longer survey that takes most of a class period).

                We hope to have all our students participate. The data will provide invaluable insights into our students’ experiences and how we can improve and adapt our school to better meet their needs. While our goal is to have 100% participation, parents may choose to not have their child participate. If you want to have your child participate, we must have a recent (within the last 4 months) consent form on file for your student. If you would like to see a copy of the questions, follow the links below.

HIGH SCHOOL KCTC Survey
USD 405 – Lyons administers the Kansas Communities that Care survey to 6th, 8th, 10th, and 12th graders with parent and student consent.  The survey is anonymous and voluntary. The survey measures risk factors and protective factors in our schools and community by totals only. The results are used by school counselors, administrators, the school board, and community organizations to detect trends so that interventions and resources can be allocated to address areas of concern.

The students in USD 405 are not surveyed in the modules pertaining to gender identity and sexual orientation.

Below are the three surveys with information, permission pages, and questions (English and Spanish) each in one document. These can be used by middle school and elementary students. The KCTC is a very big document, approximately 50 pages, because of the questions that are possible. These documents are readable/printable versions.
