2011-2012 Park Enrollment Forms

The "Main Enrollment Form" is for new students to our district only. Complete those 2 pages AND the one titled "Student Emergency Contacts & Health"...(NEW STUDENTS COMPLETE BOTH FORMS).  

ALL students and parents are asked to read, sign and return the Technology Acceptable Use Guidelines form provided below before students will be able to use the computers. Also included Park's School Supply list.

The students who have already been in our district should complete and print out only the form entitled Student Emergency Contacts & Health.  Other forms will be handed out at enrollment.
