Lyons Middle School 2011-2012 Enrollment Forms
The "Main Enrollment Form" is for new students to our district only. Complete those two pages AND the one titled "Consent & Acknowledgement Form"...(NEW STUDENTS COMPLETE BOTH FORMS). ALL students and parents are asked to read, sign and return the Technology Acceptable Use Guidelines form provided below before students will be able to use the laptops.
The students who have already been in our district should complete and print out only the form entitled "Consent & Acknowledgement". Other forms will be handed out at enrollment.
