Central Elementary School 2011-2012 Enrollment Forms
The "Main Enrollment Form" is for new students to our district only. Complete those 2 pages AND the one titled "Student Emergency Contacts & Health"...(NEW STUDENTS COMPLETE BOTH FORMS). ALL students and parents are asked to read, sign and return the Technology Acceptable Use Guidelines form provided below before students will be able to use the laptops. Also included Central's School Supply list.
The students who have already been in our district should complete and print out only the form entitled Student Emergency Contacts & Health. Other forms will be handed out at enrollment. We will be mailing a letter to each student on June 26th with a 'welcome back' your teacher is.... and the student demographics page (information already in IC) with a note to make any changes and bring with them to enrollment as well as the Student Emergency/Health pages to complete and bring to enrollment.
Please keep in mind there is a $20.00 Technology Fee for student use of laptops at Central.
