The 2011-2011 School Supply List

To begin their day, students may choose to have breakfast in the LMS cafeteria using their school meal funds or walk before school in our gymnasium. The breakfast and walking program are from 7:30 to 8:00 A.M. Keep in mind the school doors are locked and no supervision until 7:30 A.M. Students are to enter the school through the EAST gym door entrance and report to the gymnasium for our morning walking program and opening assembly. At 8:05 the Pledge of Allegiance is recited followed by a moment of silence and any needed announcements. Students report to their classrooms with teachers between 8:08 and 8:10. A student is counted tardy at 8:10. School hours will be 8:10 to 3:25 Monday through Thursday. Classes will dismiss at 2:25 on Fridays.

Many of our online forms and other information will be in PDF format. For a free download go to Adobe Reader

Valuable instruction time is lost when a child is tardy or absent. If your student is absent or tardy for more than 3 hours during the day, they will be counted absent for the day. Three tardies results in a parent letter, subsequent tardiness will require make up time after school. A student that is absent for 3 consecutive days or 5 school days in a semester without a valid reason will be considered truant. If your child will be absent or tardy, please contact the school office at 257-5612 by 9:00AM or e-mail cboroughs@usd405.com. If the office is not contacted, parents will be called or receive a home visit concerning students attendance.

Students who are enrolling for the first time in USD #405 - Lyons should bring immunization records, birth certificate and social security card. Forms and other information can be found with the link on the left margin.

A reminder to all visitors. All doors to the building will be locked during the school day, except for the front doors located on the South side of the building. You are required to stop in the office and pick up a visitor's badge to wear while in the building. This assures the staff the proper check-in procedure has been followed. An outside doorbell has been installed near the front doors if visitors wish to enter our school and find the front doors locked. Please use the doorbell and someone will come to assist you. Your cooperation in this matter will be greatly appreciated.
